Patients are often nonadherent without telling their clinicians, which can lead to unnecessary escalation of therapy and avoidable harm. Because nonadherence is so common and often hidden, the first step is simply asking patients in a nonjudgmental way. Creating a shame-free environment helps uncover barriers and improve treatment decisions.
